How to Document a Daycare Incident Report (With a Parent Signature)
A child trips on the playground and scrapes a knee. You clean it up, give a hug, apply a bandage, and the day keeps moving. Snack is in twenty minutes and someone needs a diaper change. The part that lingers is not the scrape. It is the paperwork: writing up what happened while it is still fresh, telling the parent before pickup so nothing feels hidden, and getting a signature that proves the family saw the report.
On paper, that means finding the right form, writing it out by hand between transitions, catching the parent at pickup while a toddler pulls on your sleeve, and then scanning or filing the signed copy somewhere you can find it months later. If the parent forgets to sign, you are chasing them tomorrow. If the form goes home in a backpack, it may never come back.

What this costs you today
The paper version of this workflow has a few predictable failure points:
- The details fade. If the write-up waits until nap time or the evening, you are reconstructing the incident from memory instead of recording it while it is fresh.
- The pickup conversation gets rushed. Explaining an incident verbally at the door, with other families arriving, is a bad time to also hunt down a pen and a signature.
- Signatures go missing. A form sent home for signing has a way of not coming back. Then you are texting reminders about a document you would rather close out.

How you solve it in CubbyBook

1. Open a new incident report
Start from Incident Reports in the sidebar, or use the Quick Actions shortcut on the dashboard when you want to get there in one tap. Create a new report and select the child involved.

2. Fill in the facts of the incident
Record the date and time of the incident, then choose the location from the fixed list: indoor play area, outdoor playground, kitchen/eating area, bathroom, nap/rest area, hallway/entrance, field trip, or other.
Then write the description of the injury or illness and how it occurred. This field is required, and it is the heart of the report. The best guideline: describe what you saw, not what you think happened. “Mia tripped on the edge of the play mat and landed on her right knee” is a record. “Mia was probably running too fast” is a guess. Stick to observations. They hold up better and they are fairer to the child.

3. Record first aid and follow-up
If you administered first aid, tick the box and record what type of first aid was given and who gave it. Then complete the follow-up fields:
- Further action taken: anything beyond first aid, like extra monitoring.
- Parent notified: whether you have already told the family, for example by phone.
- Child’s participation after the incident: full participation, limited participation, resting/observing, sent home, or taken to hospital/physician.
You can also attach an optional photo of the injury. The form tells you plainly that the photo is not included in the PDF. It stays with the report record, which keeps the printed document clean while preserving the visual detail for your own file.
4. Sign and submit the report (Pro)
Before the report goes anywhere, you sign it. CubbyBook offers your saved profile signature by default, or you can choose Draw a different signature if you want to sign fresh for this report.
Submit, and the report’s status becomes Pending Acknowledgment. On the family’s side it shows as “Needs signature.”

5. Let the parent acknowledge and sign
You do not have to catch anyone at pickup. The moment you submit:
- Every guardian is emailed immediately with a direct link to the report.
- The report card is pinned to the top of the parent feed with an Acknowledge & Sign button, and it stays pinned until it is signed.
- An Incident Reference entry is automatically written into the child’s daily log, so the day’s record and the incident report point to each other.
The parent opens the report, reads it, draws their signature with a finger, and chooses Sign Report. Parents draw their signature every time (there is no saved parent signature) and CubbyBook records the signature along with the parent’s full name and the date and time of signing.

6. Download the signed PDF
When the parent signs, the status changes to Signed and the PDF (titled “Accident / Incident Report”) is generated at that moment. Both you and the family can download it. That PDF, with both signatures and timestamps, is the document you keep on file.

What changes
The same scraped knee now produces a written, signed, timestamped record without a single piece of paper, and without you standing at the door with a clipboard. The parent gets the full written account by email before or shortly after pickup, so the conversation at the door can be human (“she’s fine, she was back on the slide in five minutes”) while the documentation happens on its own track.
A few things worth knowing so nothing surprises you:
Tip: You can correct a report only while it is Pending Acknowledgment, and only until the parent signs. Corrections require a written reason. The family is not re-notified, but both sides see a “Corrected after submission” banner on the report, so the change is visible, not silent.
Tip: Once a report is signed, it is locked. If you spot an error after signing, the fix is a conversation and, if needed, a new report, not an edit. Review your description before you submit.
Tip: You will not get a notification when a parent signs. Instead, watch the count next to Incident Reports in the sidebar. It drops as reports get signed. If a report sits unsigned, the pinned card keeps nudging the family every time they open their feed.
Tip: Only filing requires Pro. Parents can view and sign reports on any plan, so a family never hits a paywall on their side of an incident.
Common questions
Do parents need to sign in person?
No. Guardians are emailed a direct link the moment you submit, and the report stays pinned at the top of their feed with Acknowledge & Sign until it is signed. Most parents can read and sign from their phone within minutes: no printed form, no pickup-time scramble.
Can I fix a mistake after I submit the report?
Yes, while the report is still Pending Acknowledgment and the parent has not yet signed. You will need to give a written reason, and the report will carry a “Corrected after submission” banner visible to both sides. After the parent signs, the report is locked permanently.
Does the injury photo appear in the PDF?
No, and the form says so when you attach it. The photo stays with the report record in CubbyBook; the generated PDF contains the written report and both signatures.
What should I write in the description?
What you observed, in plain language: what happened, where, and what you did. Avoid speculation about causes you did not see. A factual description protects you, the child, and the family far better than an interpretation does.
